Nevertheless, over the past several decades, a better understanding of the anatomic and functional features of the esophagogastric region has emerged. We will (1 ) review the anatomy of the esophagogastric region; (2) offer a practical classification of the lower esophageal rings; (3) thoroughly review the lower esophageal mucosal ring; and (4) discuss the radiographic features of these rings and their differential diagnosis.

Pathophysiology of GERD: Lower Esophageal Sphincter Defects
44 INTRODUCTION T he most common cause of gastroesophageal reflux disease (GERD) is lower esophageal sphincter (LES) dysfunction. The LES is a 3–4 cm high-pressure zone of muscular activity in the distal esophagus. The LES is anatomically and histologically distinct from the esophageal body. Manometric and radiographic studies can be used to identify the LES. Intraluminal manometry can detect t...
متن کاملGastro-esophageal reflux and hiatus hernia--endoscopy.
The endoscopic diagnosis of hiatal hernia (sliding type) relies on direct and indirect criteria during the oesophagoscopic or gastroscopic approach. A wide separation between the anatomic and mucosal oesophago-gastric junction, the presence of a so-called Schatzki-ring, a wandering junction with sliding gastric mucosal folds during inspiration and two ringlike structures are important. Whereas ...
متن کاملDebatable points in the anatomy of the lower oesophagus.
Although there are a number of accounts of the anatomy of the distal oesophagus in the literature, none is definitive. The arrangement of the muscle fibres, the structure and nature of the sphincter mechanism, the mode of attachment of the mucosa to the muscle, the cause of the 'mucosal junction' ring, the factors which keep the stomach in the abdomen, and the anatomical criteria of a sliding h...
متن کاملLaparoscopic hiatal hernia repair. Is the mesh hiatoplasty justified?
AIM Laparoscopic Nissen fundoplication represents the gold standard in GERD therapy, although, a frequent failure of this primary repair is represented by the breakdown of the hiatoplasty. Aim of our work is to evaluate if ultrastructural alterations of the diaphragmatic pillars in patients with hiatal hernia, can explain the physiopathology of hernia recurrence. MATERIAL OF STUDY The patient...
متن کاملThe modifications suffered by the peri-esophageal anatomical structures in the hiatal hernia disease: a qualitative and quantitative microanatomic study.
This paper presents the results of a surgical and histopathological study regarding the microanatomical modifications of the thoraco-abdominal diaphragm and of the peri-esophageal structures in the hiatal hernias, establishing a cause-effect relationship between these structural modifications and the hiatal hernia. The study was represented by two groups of patients: a control group and a group...
